What to expect for the first visit? Physical exam, review previous lab, review current medication, review supplement list, review medical prescription and possible creating a treatment plan (depending on the case).

Download and complete Magnolia Personalized Medicine Forms prior to consultation (found above). You can either bring with you for the first appointment
You may bring recent and pertinent labs tests results with you or e fax them.
We do not accept any private information forms send through e mail.

Initial visit is at least 60 minutes and can be extended to 90 minutes.The follow up consultations can be anywhere from 60 minutes, 45 minutes, 30, 15 minutes depending on the case. Please, fill out the initial intake (found above) prior to the appointment time. You may fax it or bring it with you.

We accept only credit cards, cash or checks.

We do not work with insurance. We do not file or bill for reimbursement purposes.

Consultation fees are due at the time of the visit and are based on time spend. Lab tests are not included with the visit. If you chose to do the Oligoscan test can be done in office the same day and the cost is $200.
